4 2 Restore LLC is a Behavioral Health Program located at 3527 N. Rolling R that offers Psychiatric Rehabilitation Program (PRP) services and Substance Use Disorder Program (SUD) services. The program includes Intensive Outpatient Program (IOP), Outpatient Program (OP), Partial Hospitalization Program (PHP), DWI/DUI Education, and Crisis Management. Services are provided to individuals ages 5-99 with qualifying diagnoses from a mental health professional. The organization helps clients improve and restore independent living and social skills necessary to support recovery, enhance ability to make informed decisions, and facilitate participation within community living. Services include identifying opportunities for socialization, education, and employment, with staff providing assistance within the home and throughout the community. Individual, group, and family sessions are available.
